not following company directions, unauthorized PTOs, unauthorized home time - deadhead 200 miles after refusing a load, poor communication, truck is in the negative, total miles are less than 10k per month, drives with wife as a passenger who is involved in every conversation and she delegates and controls everything he does. Lewis alone has the potential to be a PHENOMENAL DRIVER. His wife ruins him and degrades him as a man and a driver, making his performance less than desirable.
